Notary Journal (Journal Return)
Notary Journal may be surrendered in person or by mail.
In Person
Make sure that the notary public is commissioned for Santa Clara County.
The following information is needed when a notary public surrenders their Notary Journal:
- Notary name
- Notary address and phone number
- Notary commission number
- Expiration date of commission
- Indicate the number of journals surrendered
- Indicate the Bond Company name
By Mail
The Notary Journal must be mailed by certified mail with return receipt to:

Attn: Business Division

Make sure that the notary public is commissioned for Santa Clara County.
The following information is needed when a Notary Public surrenders their Notary Journal:
- Notary name
- Notary address and phone number
- Notary commission number
- Expiration date of commission
- Indicate the number of journals surrendered
- Indicate the Bond Company name
- Provide a self-addressed stamped envelope so we can return the Notary Journal Receipt form.
