Passport Requirements / Schedule an Appointment

Passport services will resume on January 3, 2022.  The latest information and updates from the U.S. Department of State Passport Services can be found by visiting travel.state.gov. 

  • Passport applications are done by appointment only.  If you are traveling within 2 weeks,  please contact the San Francisco Passport Agency.
  • Passport hours are from 8:30 a.m. to 2:30 p.m., Monday through Friday.
  • Please read through the information below then click the "+" sign in the "I have read the Requirements / Checklist" box for options to schedule an appointment.

Passport Requirements / Checklist

Children under 16 years old

​Both parents or legal guardian need to appear in person.  Please see the U.S. Department of State web site for allowable exceptions.

Also read the "Special Requirements for Children' section on the DS-11 form.

Passport Application

​Form DS-11 is available at any of the following:

​Passport Photos

​One 2" x 2" photo that must be taken within the last six (6) months.  Front view with a white or off-white background.

Passport photo service is also available at the Clerk-Recorder's Office.

Passport photos of childrenThe child must be able to stand without assistance in order to take a picture. If assistance is required, the clerk will not be able to take the picture due to time constraint.

Evidence of citizenship

​Documents will be submitted with the application and will be returned with the newly issued passport:

  • A certified copy of a birth certificate (California and Texas Birth abstracts are not acceptable).

  • A Certificate of Naturalization or of Citizenship

  • A fully valid, undamaged U.S. passport (may be expired)
  • A Consular Report of Birth Abroad

​Identification

The card must be current and government issued containing your signature and photograph or physical description:

  • A valid driver's license

  • A U.S. Passport

  • An official military identification card

  • A Certificate of Naturalization or of Citizenship​

​Acceptable forms
of payment

The following are acceptable forms of payment for fees:​

  • Check(s)

  • Money Order

    Who should I make payments to?

    • One check or Money Order per applicant to the U.S. Department of State

    • One check, Money Order, or cash payable to the Clerk-Recorder's Office. 

      Checks cannot be combined.

What Types of Services do you have?

​Routine Service

​10 - 13 weeks processing time.

​Expedite Service

7 - 9 weeks processing time. Expedite fee required. Please contact San Francisco Passport Agency if you need passports sooner.

     

    Due to high demand, there is limited availability for passport appointments.  You may consider alternative passport processing  locations for appointments.
